LONG TERM CARE & COMMUNITY

SERVICES

As a way of enhancing our services, we continually create and foster dedicated partnerships with numerous organizations within our community.

Professional nursing staff are onsite 24-hours a day. Personal Support Workers (PSW’s) provide hands-on care, assisting residents with the essential activities of daily living. At Villa Colombo Vaughan we continuously strive to meet the care needs of our residents, families and our community. Our professional staff, which includes Nurse Practitioner, Occupational Therapist, Physiotherapist, Recreation Therapists, Registered Nurses, and Register Practical Nurses, Dietitian, Pharmacist, Speech Language Pathologist and a Social Worker provide care for individuals with many complex needs in partnership with families. Our residents feel confident that our specialized and compassionate group of professionals will manage their health and social needs.

Resident Care goals are created in collaboration with the resident, family members and members of the interdisciplinary team. Desired levels of independence, dignity, and personal preference are all part of the planning process, making each resident feel welcome and at home, while meeting their specific needs. Individuals with Alzheimer’s disease and related dementias have the opportunity to live life to its fullest in a safe, supportive and caring environment.

Villa Colombo Vaughan is also home to a number of previous Vita Community members. We continue to partner with Vita who provide ongoing support to the residents and play a significant role in assisting in the transition of care to long termcare.

Villa Colombo Vaughan strives to support our residents through person-centered care. With the assistance of the BSRT, we will be able to focus on supporting the residents, families and staff in regards to responsive behaviours. We offer a secure unit for those residents who require an environment conducive to their care needs.

Villa Colombo Vaughan prides itself on providing nutritious, Italian-style, home-cooked meals. Our residents enjoy fresh baked bread delivered daily by a local baker, a grilled French-cut veal chop, asparagus or mushroom risotto just to mention a few. But what makes meal time special at Villa Colombo Vaughan is that residents are able to enjoy a glass of wine from Vineland Estates Winery with their meal (with physician approval). Daily in Piazza De Gasperis is where you will see our residents enjoying their espresso. Special holiday meals are provided consistent with the food traditions of the Italian culture.

We have an established food committee consisting of a dedicated group of residents who meet regularly to discuss and review the menu. Changes to our menus are made as a result of the feedback that we receive from our Resident Food Committee.

Our in-house registered dietitian assesses the nutritional needs of our residents. In co-operation with our residents, families and registered team members, our dietitian develops the best-suited nutritional plan for each resident.

Our home offers programming seven days a week, including holidays. These programs are designed to meet our residents’ physical, social, emotional, intellectual, and spiritual needs.

The physiotherapy team and the recreation department work together to help increase the strength, feelings of independence and self worth of our residents.

Recreational programming includes: exercise, bocce, bingo, baking, mental aerobics, crafts, garden club, cards games, movie nights and much more. Special events are designed to include families as well, like our annual family Easter gathering, Grandparent’s Day celebration, Santa’s Day and many open houses throughout the year.

An integral part of our interdisciplinary team is our social worker. Our social worker is available for residents and families in need of counseling, support, information on government funding, finances, power of attorney, education or help with adjusting to their new home.

Social work services truly enhance the quality of life for the people who live, visit and work at Villa Colombo Vaughan.

Our Chapel “Cappella San Nicola” seats more than 90 people. The residents’ spiritual needs are supported by a variety of different activities. Mass is celebrated weekly, as well as on special holidays and for memorial services. For those residents who cannot be present in the chapel, we provide them the opportunity to watch the mass live on their TV on a dedicated channel in the comfort of their room.

We offer a fully equipped, wheelchair accessible Salon where a beautician is able to meet our residents’ hairstyling needs. Appointments can be arranged through the nursing team.

Volunteers are an integral part of our team. They support residents by assisting with scheduled recreation programs, events and one-on-one interactions. Volunteers are clearly identified while onsite and trained in the roles they assume. Our home is fortunate to receive thousands of hours of volunteer support each and every year.

A pledge of confidentiality and resident safety are key components of our volunteer orientation program.
